What is the field asking?

At reception or in an office you often hear the fields as questions – usually politely with usted: ¿Cómo se llama? → Nombre, ¿Cuántos años tiene? → Edad, ¿De dónde es? → Nacionalidad, ¿Dónde vive? → Dirección.

Country or nationality?

The field País wants the country: Alemania, Panamá. The field Nacionalidad wants the adjective – and in Spanish it is written with a small letter: alemán, mexicana.

The ending matches the person

Nationality, job and marital status agree with the person you are filling in the form for: a man is alemán, médico, soltero – a woman alemana, médica, soltera.
